About the Board’s forms
The Landlord and Tenant Board (the Board) has designed a number of forms for landlords and tenants to use for various purposes under the Residential Tenancies Act. The forms are organized on this site in the following groups. Click on the heading to see a list of the forms within that group.
- Landlord Applications – these are the forms a landlord must use to make an application to the Board.
- Tenant Applications – these are the forms a tenant must use to make an application to the Board.
- Notices of Termination and Agreement Forms – these are the forms a landlord or a tenant must use to give a notice to end the tenancy.
- Notices of Rent Increase – these are the forms a landlord must use to tell a tenant that the rent will increase.
- Other forms – these are the affidavit, certificate of service, and various motion forms that landlords and tenants may need at various points in the Board’s proceedings or for making other requests to the Board.

You can view and complete the Board’s forms on screen and print the forms if you have Adobe Acrobat Reader installed on your computer. Acrobat Reader can be downloaded free of charge from the Adobe website. For frequent users
Frequent users of the Board’s forms may prefer to use the Board’s Merge Program; using Microsoft Excel and Adobe Acrobat Reader, the Merge Program allows the user to complete and print forms.
